Looks like a JAMMA AES hack job. Not unheard of BitD as an AES cart was far cheaper for an OP to buy than an MVS cart. Since MVS carts originally cost 2x-4x the price of an AES cart it wasn't unheard of for this to happen so they could run the cheaper AES carts.

What does it look like inside? I'm assuming the bios was replaced.

I always find it cool that there were people with the knowledge to do this back in the day, even though it was or nefarious reasons.
