Either Batrider or Garegga. If you're looking for fun, go Batrider. If you're looking for pain, go Garegga.

Bakraid is the most approachable for casual/survival, weirdest for scoring
Batrider is the most absurd, with number of playable characters and secret bosses
Garegga is the overall the best (shmup ever?), most well rounded, best stage design, art, and music
Ibara is most far out there in terms of weapon system, use of bombs, and most cave-like patterns

If you're a Cave fan and don't want to buy the PCB, this is your best option; worth every penny. Great games!
