I would have to say that it would be the Last Blade US version. It wasn't officially released, nor did it come with a box or case for it.

There actually were some boxes manufactured for Laste Blade, Faselei! and I think Dynamite Slugger. I'm not sure whether these were prototypes or actual production versions. SNK-USA was going to throw them out, but someone managed to save some (or maybe all of them if they were just prototypes). I have no idea what became of them after that.
It'd be pretty nice to put a US copy of Last Blade together with a US box.
As far as I know, no manuals were printed.

About how many US Last Blade carts were even produced? Seems a few made it out to at least two vendors.
I'd still say Pocket Reversi is the rarest. Even after the Edge article, we've only seen two posted on eBay for any period of time. If there really more than 50 still in circulation, we definitely would've seen more.

Not really, as the UK versions have always been more of a collector's item (yes there is a JP version btw). Also since Pocket Reversi UK is still considered by many to be the rarest release you've got a lot of people who see it and say, "OMG OMG it's really rare so I have to get it cuz it must be the best game EVAR!!!"
