Does anyone know how to wire up a 1066-in-1 board for 6-button play?

Hello, everybody.

I have a 1066-in-1 PCB that I am trying to wire up for six button play. Does anyone know how this is done? I don't see anyplace on the PCB to connect a kick harness of sorts.

I appreciate any assistance in the matter. Thank you.

According to this manual, there's an extra connector for buttons 5 and 6 on the top board.
