That game is Oohanabi, although you probably didn't recognize it due to the different packaging. Oohanabi has it's normal release and then the one you pictured. It has been speculated on before, and the general concensus was that the repackaged version (your pic) was made for Pachislot parlors as a prize to give out.

Yep, Oohanabi....the only good thing about this game is it will keep anyone from touching the pocket while it is in there...my kids won't even touch it to change the game...
it sucks...glad I only paid 6$ for it...but that 6$ would have been better spent on a 6 pack of beer.

thats pretty much the only game that shows up in complete form fairly often on ebay so i figured it was a stinker
