4 slot sound problems

I'm having trouble with the sound "motor boating." Sometimes the HA13001 blows up . Any help would be great.

Welcome to the forum snkprovida

Check the following

Correct 12 volt vcc
Broken trace somewhere around the HA 13001, ground perhaps.
Maybe a dried out electro cap in the output stage
