I am working on a consolized 2 slot. Shoud I put fuses on the 5 and 12 volt lines, or is it not necessary? If it is a good idea to include them, what amp ratings?

Not really necessary imho.
cabs do not have secondary fuses, no reason why consols should.
Anyway for those realy anal about it, typical fuse ratings would be say 6, or 6.3 amps which is a common value for the 5 volt rail and 1.5amp/12 volts. (quick blow type)

No reason... Jameco PSU doesn't put out much more Juice than the board requires.
