I'd make sure the cart boards shake..
Sometimes if the case is pressing on the boards to much it warps them..
It's hard to see this but if you can't move the carts boards at all it's possible the case it too tight on them and warping them.
Look for cracks or bad soldering/dry joints..
I'd place the cart at different hieghts in the motherboard.
And I'd take the boards out of the case and place each one in the motherboard out of the case..making sure they are put in as if they were in the case.
Does it have any EPROMS..?
If I have severe problems with a cart I clean the prongs with very fine sand paper.
I also clean the mother boards prongs too because each cart has different spots
it connects at when place in the mother board.

wonder if Clearing the back-up memory on the motherboard would help..
Or if the power supply just is a bit to low to work this cart...
